Supportive Services Case Manager | United House Apartments


The Supportive Services Team Leader will act as the point person for the coordination of Supportive Services to residents of United House Apartments, a Permanent Supportive Housing facility managed by Cardinal Capital.  United House Apartments, located at 2500 West Center Street, provides 24 little/no income individuals with quality apartments on the north side of Milwaukee.  The development targets individuals that live with mental health, substance abuse, chronic homelessness and /or homelessness and/or are prone to or at risk of homelessness, faced with a disability and/or are in need of supportive services to maintain housing stability.  

 The Supportive Services Team Leader will work with resident’s case manager(s) as well as being tasked with oversight of the various supportive services provided by a team of JusticePoint peer specialists working in the building.


  • Associate or Bachelor’s degree with equivalent field experience in the delivery and/or oversight of supportive services or case management;

  • Previous experience with Trauma Informed Care programming/interventions preferred;

  • Previous experience with Cognitive Behavioral Programming/interventions preferred;

  • High level of skill on organization and attention to detail as well as an ability to work well independently;

  • High level of computer literacy including data entry, web searching, and word processing;

  • Excellent verbal and written communication skills;

  • Prior experience working with individuals from diverse economic, racial, and ethnic backgrounds;

  • Pre-employment background check is required.


  • Review and update staff training procedures and training materials related to United House;

  • Primary responsibility for coordinating initial staff training and orientation for new program staff and ongoing training for existing program staff;

  • Review and maintenance of internal documentation and program protocols;

  • Manage and update all forms and other documents needed to track program participant progress as required per the United House Program Description;

  • Maintain accurate resident files and participate in weekly utilization reviews;

  • Maintain and provide all reports related to expected outputs required by the Milwaukee County Housing Division;

  • Provide ongoing review and quality assurance on all program services, casework documentation, and reports.


  • Permanent Full-Time (40 hours/week)

  • Reports to EI Unit Director or Designee
