POSITION TITLE: Youth Wellness Center Case Manager


Provide skill-based programming that is evidence-based to address a youth’s criminogenic needs either in a center setting or as a program for individual or small groups of youth. The YWC/YWP programming will be individualized to meet the needs of the youth referred, based on the youth’s assessed risk level, needs and supervision category.


·        Provide YWC enrolled youth with individualized programming to address their identified criminogenic needs

·        Maintain data on each program participant

·        Collaborate with local agencies involved with the juvenile and his/her family

·        Utilize evidence based programming in order to address the criminogenic needs of each youth in the YWC Program

·        Maintain a strength-based approach

·        Provide supervision at the appropriate individualized level

·        Provide meals and snacks to youth during YWC programming

·        Provide transportation to youth in the YWC

·        Be available 5 days/week to serve program participants

EDUCATION: Bachelor’s degree in Criminal Justice, Social Work, Psychology, or related human service field required


·        Computer literacy including working knowledge and experience of Windows and word processing, as well as data entry experience (Microsoft Word & Access)

·        Excellent verbal and written communication skills

·        High level of skill in organization and attention to detail and accuracy as well as ability to handle multiple tasks at one time

·        The ability to work independently

·        Demonstrated ability to work well with individuals from diverse economic, racial, and ethnic backgrounds

·        Valid Wisconsin driver’s license and automobile with insurance required
