POSITION TITLE: Youth Tracking Case Manager



Provide supervision and evidence-based programming to each youth referred to the Youth Tracking Program to meet the needs of the youth based on the youth’s assessed risk level, needs and supervision category. Tracking may include the use of GPS or home detention rules, but the level of tracking is determined on an individual basis.


·        Develop individualized case plans to address a youth’s criminogenic needs

·        Maintain data on each program participant

·        Collaborate with local agencies involved with the juvenile and his/her family

·        Assist with crisis de-escalation situations that arise with program participants

·        Utilize evidence based programming in order to address the criminogenic needs of each youth in the Youth Tracking Program

·        Maintain a strength-based approach

·        Provide tracking and supervision at the appropriate individualized level

·        Be available 7 days/week from 9am-9pm to serve program participants

EDUCATION: Bachelor’s degree in Criminal Justice, Social Work, Psychology, or related human service field required


·        Computer literacy including working knowledge and experience of Windows and word processing, as well as data entry experience (Microsoft Word & Access)

·        Excellent verbal and written communication skills

·        High level of skill in organization and attention to detail and accuracy as well as ability to handle multiple tasks at one time

·        The ability to work independently

·        Demonstrated ability to work well with individuals from diverse economic, racial, and ethnic backgrounds

·        Valid Wisconsin driver’s license and automobile with insurance required

 REPORTS TO: JDAP Program Director
