Position Title: Program Coordinator–Comprehensive Community Services (CCS) Care Coordination Team

Reports to: CCS Program Director

 Job Description

The Program Coordinator is responsible for assisting the Program Director with the day-to-day program operations including staff supervision, training, and quality assurance measures.


  • Develop and update program policies and procedures

  • Develop, and update staff training materials, program forms, and other program documents

  • Provide ongoing staff training, coaching, skill development, and performance evaluation

  • Oversee services provided by staff to ensure that clients are receiving quality services

  • Provide ongoing review and quality assurance on all program services and documentation

  • Provide coverage for staff, as needed, up to and including maintaining a small care coordination caseload

  • Review and approve staff leave requests and time entry

  • Assist with hiring of new staff

  • Other duties as assigned

Knowledge, Skills and Abilities:

  • Proficient in computer applications including Microsoft Office Suite (Word, Excel, PowerPoint) and web enabled databases

  • Excellent oral and written communication skills

  • Excellent organization and time management skills, attention to detail and accuracy, and ability to prioritize multiple tasks.

  • Ability to work independently and as part of a team

  • Demonstrated ability to work well with individuals from diverse economic, racial, and ethnic backgrounds. 

  • Knowledge and understanding of mental illness, substance use, and the effects of poverty on various socioeconomic social groups

  • Knowledge of community mental health and substance use treatment resources

  • Leadership skills and ability to provide effective staff coaching, training, and team building

Minimum Qualifications:

  • Must possess a B.A. or B.S. degree in Social Work, Psychology, Nursing, or a related field with experience in human services and case management, Master’s degree preferred.

  • Prior experience in leading or managing staff in a human service program preferred

  • Valid Wisconsin driver’s license and automobile with insurance required.
