POSITION DESCRIPTION

 

Position Title:                  Comprehensive Community Services (CCS) Care Coordinator

Program:                         Comprehensive Community Services

Reports to:                      CCS Service Director
 

Position Summary:

This position is responsible for helping the consumers to understand, navigate, and grow through the CCS program.  This Care Coordinator meets and works with the consumers, service providers, and other team supports that assist in the consumer’s progression through the CCS program.  The Care Coordinator receives consultation from mental health and/or substance abuse professionals while working on the clients individualized recovery plan. 

Position Functions:

  • Establish client eligibility by using a Mental Health/AODA Functional Screen
  • Assist the clients in identifying services from the CCS service array appropriate to address barriers to recovery
  • Work with the client to identify and build a recovery support team of professionals as well as natural supports
  • Broker services as needed with other providers in the CCS network
  • Work with the clients to make an individualized recovery treatment plan
  • Work with the client to prioritize and obtain the goals set forth in the individual recovery plan
  • Facilitate recovery meetings with the client and the team
  • Follow-up with service providers to ensure quality care and goal progression
  • Complete necessary authorizations and paperwork
  • Appropriately document all contact with clients and team members

Knowledge, Skills and Abilities:

  • Proficient in utilizing Microsoft Office applications including Word and Excel and web enabled databases
  • High level of skill in organization, attention to detail and accuracy as well as ability to handle multiple tasks at one time
  • Clinical knowledge and skills with the ability to clearly communicate these to others
  • Ability to cope with emergency situations
  • Ability to maintain and promote positive working relationships with others
  • Excellent assessment and interviewing skills
  • Good understanding of mental illness and its effects on consumers and their families
  • Good understanding of the AODA population
  • Effective use of time and resources to help maintain quality CCS services 
  • Ability to interact effectively and sensitively with individuals from diverse social, economic, and ethnic backgrounds
  • Exercise good judgment
  • Experience working with Milwaukee County Behavioral Health Services preferred

Minimum Qualifications

  • Bachelor’s Degree in Social Work or a related field; Master’s Degree preferred
  • One (1) year of experience working in the Mental Health field preferred
  • Experience working with substance abuse client’s preferred
  • Must complete a criminal background check
  • Must have valid driver's license and insured automobile
